What is the form you are referring to? Based on this form http://www.jotform.co/form/31276410074850, I checked the recipient for it's notification, and our mail logs shows good delivery.
Have you checked the spam folder? You may white-list our domain names, if the issue continues, as we use Amazon SES method for "email@example.com" sender:
