Access Denied

I was told to include a screen shot of the error message I am receiving. Below is what I am receiving and I have filled out this form twice. 

 

It seems that the IP address is still banned by JotForm. Can you provide us the IP address of the device you are using where you encounter this error? You can get your IP address using this link https://whatismyipaddress.com/ so we can whitelist it. Access the link using the device where you encounter this error.
