Form Answers

    Hello, I have a paper form in a format with specific questions and answers fields on each row.  When a user completes the form and submits it, I believe that I would receive the answers to each field in a list going down by email.  Is it possible to create a form where I receive the completed form in the exact same setup as the original form.  For example, let's say I have the first line ask for First Name, Last Name, Phone Number.  When I receive the answers to the form, can the answers appear all on one line?  Or will they appear on 3 separate lines since there are 3 questions?  Thank you.

    What you would like to achieve with your form is possible. What you need to do is arrange the content of the email that you will receive when your form is submitted.

    Here's how you can do that. 

    But, of course, you can arrange it to your preference.
