Also if I copy and paste the html code to the website it just shows up blank.
I would like to apologize for any inconvenience. I have checked your account and verified that it is auto-suspended. We have an anti-phishing system that reviews every form on every account. When a form is suspected of any phishing activity it will put the account on temporary auto-suspension status. I have reviewed the forms on your account and have not seen anything suspicious. I have reactivated your account and everything should work now.
