I submitted a registration form for my own son to make sure the form was working. I was able to see the completed form on jotform.com but no emial notification was sent to me informing me of a completed form.

I checked your forms notification and there is not currently a recipient address for the email:
Since there is no address for email to be sent to, it is not being sent. Update the recipient email to your address and the email should send properly once again.
