Are you referring to your form name/page title in the browser tab? If yes, since the form is embedded, you can't see it there. You'll only see the Facebook tab/app name. If not, please share us with us your Facebook page and let us know what name are you referring to. A screenshot would be helpful.

I'm afraid your image didn't get through, since our forum software cannot accept inline images sent through email. In order to attach an image, you would have to open your support thread directly in a web browser, and attach them in a manner described here:

When it comes to the issue at hand, without seeing the image, I assume it may be related to the Page Title setting of your form. There is a difference between the name of the form, and its Page Title (HTML title).
You can change the Page Title by going to Preferences:
Once there, switch to the Advanced tab, slick on Show More, and change the Page Title to the name you want to see when the form is opened directly in a web browser:
