    Error in HTML.

    There are " and > characters at the top of the form.  I'm using WordPress, but I noticed it also appears in another tool.

    Can you please fix?

    It seems that your form 52527344140145 got saved with HTML tags inside the form's title, and these HTML tags are now causing some disruption on the rest of the form.

    The current name of the form is this:

    <input class="edit-title" value="LMBA CAMPS/CLINICS/TRYOUT REGISTRATION" type="text">

    Please open your form for editing in our Form Builder, and make sure to change the form's title to something else. I believe this was the intended title:

    LMBA CAMPS/CLINICS/TRYOUT REGISTRATION

    Upon correcting the title of the form to no longer contain any HTML tags, and saving the change, your form should display correctly. Please let us know how it goes.
