I'm afraid we couldn't view the link you've shared, because it automatically redirects us to a login prompt in order to access it:

Upon selecting to view it as an unregistered user, no file was shown:

In order to have your users fill out a form from which you can receive responses, you must build your entire desired form with our easy-to-use Form Builder:

    https://www.jotform.com/help/2-How-to-Create-Your-First-Web-Form

Our form cannot collect form responses from other forms, or other files. If you want to display a PDF file on your form, you can indeed display it on the JotForm form by embedding it in the following manner, but your embedded PDF file itself will not be forwarding any collected data to JotForm:

    https://www.jotform.com/help/153-How-to-Display-a-PDF-Document-On-Your-Form

This means that if you want to receive responses to questions that you may have inside a PDF file, you will need to create such questions on your JotForm form instead, and collect your responses through the JotForm form.
