Form Editor losing changes, adding deleted elements back to form

For the second time today, the form editor has "lost" changes I've made; conditions, widgets, items on the form, etc.

I am 100% certain that I saved. 100%. I even previewed after I saved. Yet, when I return to my form builder tab, which I never closed or navigated away from, things have disappeared, deleted items reappear, and worst of all, I lost part of my Conditions logic. Twice!

The attached screenshot shows the conditions I had earlier. I lost 1/2 of them.

I'm using Firefox and have multiple tabs open (for other sites, only one tab for form builder).

I don't have a published form yet. I am very frustrated. I'm almost ready to publish, and I'll need to convert my free account to a paid account, but not if this keeps occurring.

What am I doing wrong? Or is there a system issue.

I am not able to find the screenshot in your post, so can you please send it again, so I can check.

Also, I have created a test Form, and I have not lost my widgets,  and conditions. 

So please provide me further details, so I can check it.
