I am attempting to embed a form into a page for a client. The page shows perfectly on the development site, http://dev.jasmineconsultingllc.com/checkup; however, when I visit the production site (which is a copy of the development site), http://www.jasmineconsultingllc.com/checkup/, the page doesn't display the form.
Also, it appears that it's taking the form much longer to load the page.

But I did notice slow load time for the page.
Can you retry also re-embedding the form using its iframe embed code instead. Follow this guide: Getting-the-form-iFrame-code
Make sure you clear/remove first the previously embedded script codes of the form before you add it back using its iframe embed.
Let us know if there are still issue.

Thanks it's working; however when testing the form it says "There are errors on the form please fix before submitting".

Are you still having the issue at this time? Can provide us with more details also if it does.
Perhaps the error message you see was just the require validation message? That iwll appear if there was a required question that was not answered.
Let us know if issue remains.
