I created a form but it does not show up on my account.

Upon checking your form #61854542163355, I found that it is associated with a guest account. This type of accounts are created if you do not sign up / sign in to your JotForm account and start building forms. 

I have moved this form to your account with username "pedroHuddle" and you should now be able to see this form in your account.
