How can I gather summarized results, multiple times?

I placed the same "If/Do" condition on fields 45-56... basically, if the drop-down answer is equal to "yes", than the value of that field ("yes") should appear in a summary box... field 57.

Then, I set up a configuration that if field 57 is filled, all the value (... does VALUE = CONTENT?? I sure hope so!) within it should appear in a summary box later on... which is field 61 in my form.

I tested the form, and selected "yes" to a number of the answers.

Based on my configuration, this should have resulted in field 57 containing multiple "Yes" statements.  However, take a look at my attached screen shots. The only value I ended up with in field 57 (which is "Notes") and then again in field 61 (which is "Atmospheric Hazards") is "0"... which actually popped up from the condition that was placed on replicating the value of "Type" into field 57... so it should say "Mist"... not "0".

    Thanks for the help!

When multiple conditions are performed on the same field, the most recent condition checked will be the one that updates.  It will not combine each condition into a single output for the field only the most recent will be displayed.

If you want to combine multiple field values into a single field, you can use an update/calculate form field value condition:

You would then combine all your fields into a Text area, similar to what was attempted with multiple conditions:

Click on the 0-9 icon to change it to A-Z.  This will interpret the field as text.  Here is a quick test form with the condition above added:
