I keep receiving read errors on the submission forms hosted on JotForm. How can this be fixed ASAP?

We apologize for any inconvenience caused. We had a brief disconnection issue on our end that causes this. But our backend team have resolve the issue now.
I test submitted one of your form, and I can no longer reproduce the error. I see the Thank you page properly.
Please check again. Do let us know if issue persist on your side.
