How can I capture Leads/Campaign source?

Hi, was wondering if it's possible to capture lead's source likewise Campaign source, Campaign Medium and all.

In short, wanted know from where my leads are coming?

On the Submission listing page, you will get the IP-address of the user who submitted the Form.

I would like to give you one suggestion, on your Form you can add either a drop-down or a Text Field as "Source". So user can enter or select the value in that "Source" field, and then you will be able to check that while checking the Submissions.
