Creating a form with upload field

Looking for a form that we can use for our photo contest (entry fees).

So, the form must be: registration > submitting (uploading photo's) > paying system > confirming message

You can browse the available registration form templates in this link: https://www.jotform.com/form-templates/category/registration. Just click on the Use Template button once you made your choice.

You can add, change, remove or customize the form fields by using the tools under Basic Fields or Quick fields.

You can insert a File Upload field on your form so that your users can upload their images.
