    There is no way for you to create a form that will look the same as the form you have provided. You will have to create the form from the Jotform components. Please take a look at our templates, they can help you achieve the similar look as the form you have provided.

    I saw the templates. A widget like 'address' would attend me. You know, various fields with a subtitle under, indicating what to fiell there.

    How can I create one?

    To add the address in your form you just need to drag the component from the tools to the form builder.

    Yea... I know.. but I want a adress like widget.. not the address widget.

    I could not customize the address widget, so I need something like that, but with my own fields..

    We do not have widget that is similar to address field. However you can customize address field with CSS code. You can also change the sub-labels of address fields. To change sub-lables simply double click on sub-label text and edit it. You can also remove some fields from Address field.

    You can also use regular text box fields and position them to look similar to table you attached. More about form field positioning you can find inside of this guides:

    sigh...

    That do not help..

    Adress is what most aproach the make that table, but it is limited to only 5 text field. I would need 11.

    I guess it would not be hard to make an adress like field, but customizable: any field number, any field type..

    You can use two Address fields and position them in same line. Here is my demo from: https://form.jotform.com/61896711019965 

    You can also use regular text box fields and position them same as fields are positioned on table you provided. I have also made demo form with text box fields. My demo form looks like this.

    Actually no, because of the fixed format. I have to customize it. And one field would be missing. I still think a good idea to create a widget similar to it.

     

    I had to create various text fields, shrinked.. and still had to edit the line css to eliminate that enormous padding or something like. A camel can walk through the fields due the pattern space!! And nothing but css works on that. This is a problem.

     

    So, I minimize the space between to get the fields closer and it helped. As I said, there is 11 fields, in three equal groups (33 fields, that should be separated in 3 small blocks). Not perfect, a little confuse but.. the only solution.

     

    I would like at least to insert a line break between these groups, but saw no option for that. Is there any way to insert a
    there?

    I would suggest you to take a look to our Configurable List widget, this is a widget that will allow you to add multiple fields inline, but you cannot add page breaks; however, this can be easily achieved by injecting some CSS code to your form so all fields will be displaying as I think you need and they will be displayed in rows.
