Is it possible to create a door prize style form with jotform?

I've got a question about whether something could be done for a contest we're wanting to set up, possibly with one of the widgets you offer? 

The concept is that you pick one of three (number variable) doors, and you get entered to win the prize behind that door.

For example, Door 1 has a $100 restaurant gift card, Door 2 has a pair of shoes from a store, and Door 3 has a pair of tickets to a football game.

We would need the ability for people to “click to enter” and end up on a landing page where they enter their name, phone, e-mail, and then click a door to find out the prize they’re entered to win.  Which prize is behind which door needs to be randomly assigned every time someone enters so that they can’t just go back and tell people to pick Door 3 because it’s the best prize.

They’d get a follow-up e-mail that says, Congratulations!  You’re entered to win XXX behind Door 3.  Thanks to all of our sponsors, etc.

Is there a mechanism/widget to accomplish this already?  Or is this custom?

It seems to me that you primarily want to create a "door prize" contest form using 3 door prizes.

In my opinion I would recommend doing this similar to creating a multi-payment form but instead of using it for payments you'd be doing prize forms instead.

Basically you would create 4 forms. The first would be the master form with the other 3 door prize forms embedded using our iframe widget on it.

You could then use something like our image radio widget or image picker widget to show one of the 3 door prize forms via conditional logic for them to enter for that particular price which would be connected to an autoresponder as a confirmation to the user entering the contest for that prize.
