Your email address is in the bounce list. I have removed it now so you shouldn't have problem receiving new notifications from JotForm.
I checked your form and I can see that it is properly configured. You might also need to whitelist our mail servers and IP addresses, here's a guide: Whitelisting JotMails IP addresses
