    We have received your question with only a title "Financial Plan Fact Finder/Investor Questionnaire". I have checked the link that there is a PDF form with the Questionnaire is available to download. Do you mean to convert this PDF file to an online form? 

    Though the form can be imported to your JotForm account to convert it as online form from the Fillable PDF form creator, due to the complexity of the form I am not sure if we can get the same result as the PDF version. However, you may give it a try to import the PDF form.

    You may also create a new form by using the intuitive drag and drop user interface in the form builder easily. Once you have the required fields in the form builder, you may create a new theme for your form by using the Form Designer or you may select a theme that matches to your form from the Theme-Store.

    I have just tested with a sample PDF conversion from the Fillable PDF form and see that the form imported is not editable. However, the received submissions from the PDF form can be stored in your JotForm account. If you need to make the form editable online, it is required to create a new form.

    I am looking for a template on JotForm that contains the same content as the form I referenced in my initial inquiry. Kindly show me where I can find it.

    I have created form that has all fields like the form on link you provided.

    Feel free to clone it. 

    File upload fields have default size set to 1024 KB (1MB). Inside of this guide you can find how to change the upload size limit: https://www.jotform.com/help/33-Changing-the-upload-size-limit 

    Also please note that upload storage space for Free account is 100 MB and that JotForm upload max size is 1GB or per file.
