Your account was autosuspended by our phishing detector due to violation of jotform's terms, which is why your forms are currently disabled. Please note that collecting user's credit card information is strictly prohibited on jotform.
To remove the suspension on your account, please delete the form titled "Credit Card Form Webroot Subscription" and avoid collecting this information in the future.
