How do I get the html and php code for my form I created?

We don't offer the embed codes in PHP. I'm assuming you are looking for the actual source code of your form. You can find that by clicking PUBLISH at the top > EMBED > click the EMBED OPTIONS dropdown > select SOURCE CODE > then COPY CODE
