How can I copy an existing form into a JotForm?

    I tried adding the URL to the form I've created but it didn't work: http://www.excelsiorhealthcenter.com/uploads/7/3/8/6/7386018/excelsior-new-client-form-sign2.pdf

    I need the client to be able to fill out the form online and email it to us. I tried doing it through an adobe acrobat file but the submit button won't work through the website. 

    Because the form is kind of complex, it would be hard to create from scratch on JotForm. Plus my client wants to be able to receive the forms as pdf files so he can print them. His clients find it too complicated to save the form to their desktop, fill it out and then upload it again as an attachment to an email and send it to him. We're just trying to make the process simpler for them.

    You can import the PDF file using our fillable PDF form creator: https://www.jotform.com/fillable-pdf-form-creator/

    Please note that the import tool will only give you the same file PDF format, not an online HTML version. You can download the PDF after and send it to your clients for them to fill. You have the option to collect the entries automatically or manually. I'm not sure if the import tool will adopt the submit button in your PDF, but I think it will create a new one.
