Display complete form along with text in the PDF submission

Hi there,

 

We have a form that we use and when we go to download the PDF it doesn't do the entire form (the top text is missing). I have checked the actual form itself and everything is on there. I'm not sure what went wrong, but this just started happening recently. 

You can see in the screen shot below the top text is missing.

If you're referring to heading and text you just need to turn it on to be shown on the submission. And you can do that by following this:

1. Go to My Forms and click Submissions:

2. And by clicking on gearbox you can choose to show heading and text:
