Still having issue with my salesforce leads integration

all forms filled as advised but still no leads

form on jotform is called
REAP Home Ownership Registration Form

I can see you have opened other threads to post the same issue, you've already got replies from our Support Reps.

Now, I have also tested your form and integrated with SalesForce like my colleagues, I can see that using the fields you're using it does not work.

But when you use the correct fields it works fine, you're currently asking for the name in separated fields, email address in a plain text field and you've not your company field.

I would suggest you to map the fields as they are in the Form Builder, so you should use the Full Name, Email address and Text Box fields to map on your integration.
