Form reverted to earlier version: Why?

    I created a new form by cloning a previously used form.  I changed the name of the page and edited details in the form.  I saved the form under a new name.  The form went live, and we got several responses.  That was yesterday afternoon and this morning.  Sometime today, the form reverted in my list to "Clone of.." and I can no longer find or reach the edited page.  

    I would be more clear, but the entire incident is weird and hard to describe.  The short version, a form reverted to an earlier version, and all the subsequent changes have been lost.  I can no longer reach the form created to edit it. Yet it remains live.  

    I have disabled the form.

    The form you referenced should be available from the "My Forms" section of your account:

    It is possible to restore the form to any recently saved state using your revision history:
