Forms are disabled

My first form is disabled for no reason i tired to enable it but it won't work

I checked your forms and I found out a fields that are asking for a password. You cannot collect login credentials or sensitive information here in JotForm, please check our terms of use. This is the reason why your account got suspended by our phishing tool.

We can activate your account but we need to delete the guilty fields. If that is okay with you, let us know. Thank you.
