Whenever we click submit. The form is not being seen in the email address supplied, but the auto responder works fine.

We are sending the form emails based on our logs. You can check your email history for more information: https://www.jotform.com/help/364-Why-I-am-not-receiving-email-notifications. Please check your spam folder for the emails and make sure to mark them as not spam.
I see that you are using a custom email address as the sender email of the form, you have it through SMTP method. You may consider asking your hosting provider if they can find some logs related to your email account. They have the control to check this for you.
Lastly, try to separate the recipients in the form into different notifiers. One notifier for each recipient. Check if that works.
