Thanks... it works great unless viewing it on a Mac with Safari...
The form doesn't show up.

The form is showing up using El Capitan and Yosemite, Safari 8 and 9. Any extension blocking the form? Can you check?
Please check your Safari console as well to see if there are errors that possibly causing this to happen.

ok... I am running Yosemite 10.10.5
Safari Version 9.1.2 (10601.7.7)
I disabled Safari extensions, and it shows up...
and it still works when extensions are turned back on...

I strongly advise to use the iFrame embed code if all plugins failed.
