Form to prefill another form

Hi,

I run a Physiotherapy business.

We get referrals from other health providers.

I want to know can I create a referral form which could be on my website whereby the client information entered could pre fill an initial assessment from which my Physiotherapists would then work with for the first assessment?

That's possible with JotForm. You can pass form value to another form.
