Why is my form not found?

Your account was suspended due to violation of jotform's terms. Please note that collecting a user's login credentials (username or password) or bank details is strictly prohibited in Jotform. Your form was found to violate this rule so your account was suspended. Here's an excerpt from our terms: 

Phishing. You agree that Company may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information
