I checked some of the pages on your site and I can confirm that the form is not displaying correctly.

I see that you are currently using the script embed to embed the forms in your wordpress site. Can you please try using the Embed Form Plugin instead? Here's a guide that may help: Embed JotForm in Wordpress using the Embed Form Plugin

If the issue persists, please let us know.

If you are logged in to your jotform account, the forms should appear when you click on the pencil icon. Otherwise, you will be asked to sign in to your jotform account in the popup window.

If you are still unable to see the form, please try clearing your browser cache and refreshing the browser. If the issue persists, please let us know.

I checked the Coaching page again and I noticed that there is an extra 0 character within the script code.

I cannot confirm if this is the cause of the form not appearing in the page, but will you be able to edit the script code again and remove the 0 at the end in your coaching page? http://petercharleston.com/coaching/

If the form still does not appear after the extra character has been removed, then you may try using this alternative plugin called Jotform oEmbed. Here's the link: https://wordpress.org/plugins/jotform-oembed/installation/.

There's also the iFrame embed code which you can try, simply follow the link here: Getting the form iFrame code.

Please let us know how it goes.
    Regards.

I am glad to know that the solution worked. You can try and embed two forms onto one page using the iFrame embed code. Please let us know if you need further assistance. Regards.
