I am not receiving any notifications that form has been submitted.
Maybe I did not fill it out correctly
Could you trouble shoot and reply to:
I have checked the email address you're using on your form and I can see it is in our bounce list, here is the reason for that:
Reason(s) : smtp; 550 5.7.1 Recipient not authorized, your IP has been found on a block list
I have removed it now and I would like to share you this guide that will help you to avoid bouncing: How-to-Prevent-Email-Bouncing-Related-Issues
Here is also a guide that will help you to check if your email address is in our bounce list, so if you stop receiving emails you can check your address here and if you find it in our bounce list you can remove it: How-to-Remove-Your-Email-Address-from-Bounce-List
Also, if the issue still persists, I would suggest you to ask your email provider to white-list our IP addresses, here is the link with our info: Whitelisting-JotMails-IP-Addresses

What's up with that?
I have checked the form you have embedded in your website and I can see the sender email address is set to the default.
If you had set up a custom sender email, maybe it was set to default because of failed configuration; however, you can set it up again, here are two guides that will help you to setup SMTP or another method to use a custom sender email address in your form.
