Emails from the form aren't being delivered. I have configured and tested an SMTP account and messages aren't even being sent that way according to an Office365 MailTrace. We have explicitly white-listed based on message headers and done everything we can to try and get messages to arrive but it appears you aren't even reliably sending them to start with.

Sorry for the inconvenience. Instead of using a custom SMTP, can you try using one of our default email address to send notification. If you are able to receive the notification with our default address, I would recommend reviewing your configuration and email server.
