TWO QUESTIONS:

1 - My Form NOT receive Mobile Photo Maker SAMSUNG GALAXY . How to solve ?

2 - I divide the fields into two pages as a solution for iphones . In iphones , page scrolling down does not let the SUBMIT button to work . When you click SUBMIT , the form back to the middle and you can not click the submit button .

If I understood you correctly Take Photo widget is not shown on your Samsung Galaxy. I have tested your form on my Samsung Galaxy S4 and that field is working correctly. Please note that Take Photo widget will be shown as File upload field on mobile devices and your users will be able to upload an image form their gallery. 

I have also cloned your form and  tested my form on iPhone simulator. I was not able to replicate mentioned issue. You have Preview Before Submit on your form. When I filled the form and clicked on Preview Before Submit button all data that I entered were shown.

After that I was able to submit the form and I got redirected to Thank you page.

Are you still unable to submit your form on your iPhone? Which iPhone are you using? Can you try to test your form on some other iPhone device?
