My form says Form Not Found and I can't embed it anywhere

The title says it all...I see the form in my forms, but I can't use it. Not with an embed...not with a link. Just doesn't work.

Your account was set to "Dormant" status, I presume the account was first registered way back in 2013 and was inactive for a long time? I have re-activated your account, your form should now be working again.
