Can't Edit or Preview form after migration

    Thanks very much for all your help.

    Sorry to bother you again  (and I'm not sure if this is due to the migrate operation or not) but my Hardwick Appointments form seems to have got lost. It appears in the main list (see screen grab below) but will not preview and EDIT shows a blank screen.

    Interestingly the date is 28th March and I did do an amendment to the form around that time.  If it could be recovered then that would be great!"

    I cloned the form from v2. Please ask him to use this new form: New Hardwick Appointment
