I have seen that you have updated the post very soon, so I think that you form jotform will work for my latest version of PrestaShop. Here I want to know about your tool, Can I use it on product pages just like offering this module http://www.fmemodules.com/en/86-product-contact.html . Also let me know that i want to show a contact form button on product page, when a user clicks on the button, then the form open in ajax popup window. Does you tool support this?

Unfortunately, JotForm's capacity to include the form to PrestaShop is limited only to having the form in it. It has no control or access to use the modules such as the link you have provided. Also, it will not be possible to use a pop-up contact form with using ajax as PrestaShop restricts the use of Javascript codes to the web pages.

But to answer whether JotForm has the capacity to do such, the answer is yes. You can use Lightbox, or Feedback embed option, but not to PrestaShop as due to its restrictions.
