I can see you've set up your custom sender email address, but I can see you are testing your email notifications using the "Test Email" button placed ni the email wizard.
Please do note that this will send the email from our default sender email address and it will always be sent to the email address set up in the account.
I would suggest you to test your form with a real submission, that way you will know if the custom sender email works or not.
Please try that and let us know how it goes.
