I have a form that was working just fine that had 6 page breaks in it. Randomly the last two page breaks are no longer working. So when you go to complete the form, after page 4 you click next and a blank screen appears. I have removed and reinstalled the page breaks for pages 5 and 6 but still the error continues. So i completely took over the page breaks for 5 and 6. Please advise on how to get this function to work again and what caused the disruption.

I'm able to replicate the issue at my end with the cloned version of your JotForm. Please allow me some time to run few tests and get back to you.

After investigating the issue, I found that the issue is being caused by the following text field in the page #4.
Please remove this text field from the form and add a new text field. Please make sure that you are not copying the text from any word document which may be causing conflicts with the CSS code of the form. If you need to, you may copy and paste the text to a notepad application and then copy from notepad to paste it in the text field.
It should be working fine after changing this text field. If the issue still persists, please let us know. We will be happy to assist.

So I completely removed text box on page #4 the issues still continues.

I cloned your form, and the issue reproduced, I deleted and add back the following text field located on page 3 under "COLLABORATION WITH OTHER STAKEHOLDERS" headers:
And that fixed the problem:
