How can I put the same fields more time in the same form

Hi, I find Jot form very good for my web site, but I need one more option to start using it.

I need to put the same fields more time in the same form. i give you an example.

A subscriber fill the form for him and his friends, he finds in the form:

Name subscr 1

Name subscr 2

Name subscr 3

In my database I will find the list in horizontal, like this:

Name subscr 1   |  Name subscr 2  |  Name subscr 3

But I need to have that names in vertical in my database:

Name subscr 1

Name subscr 2

Name subscr 3

The subrscribers has to fill the form one time in one page, not more times.

Can you help me?

All fields are all vertical/wrap in the form submissions page as well as in the email. Perhaps you're looking for something different. Do you mean the field options? Perhaps the checkbox, radio button, and dropdown fields?
