Hi,
I have created a test form and I checked in the mobile phone,some field went away from the form layout boundary and not properly aligned .Please check the form link below and help me to resolve the mobile phone view and how to align the fields aligned properly.

You can add the Mobile Responsive widget to your form to resolve the issue with mobile responsiveness. Here's a guide that can help you set it up: How to Add a Widget to your Form.
Once added, the form should look like the screenshot below.
