I connect Jot form to Active Campaign through Zapier. Contact is not adding to active campaign list after submitting form successfully. I want contact automatically save to active campaign list when anyone submit form.

I cloned your form and connected to Active campaign through Zapier. And everything seems fine, my contact transferred without any issues:
Make sure you're connecting to the right campaign, also that fields from jotform are matching correctly to the Active campaign.
What you should keep in mind is that Zapier is checking new submissions every 15 minutes and then it pushes the new information to Active campaign.
Please check your fields mapping and let us know if the issues persists.
