I want my employees to feel comfortable giving honest answers because it

Thank you for the additional information.
The only way to see who submitted a form, would be to add a name field and ask the end user to enter their names. While reviewing your form, Employee Survey, I did not see any fields asking for personal information.
There would be no way to really identify which user submitted what form. You can probably trace the IP address, but this would not guarantee you would find the user. Many users could submit the form from the same device.
