How to set up submitter's email address as sender?

Is there a way i can set up a form that can be submitted by different people so it comes from those peoples e-mail addresses , for example we have new starter forms that could be filled in by various people and when the form hits our ticketing system it uses the sender e-mail address to log it under that persons name.

I can get this set up using add sender e-mail option but that's only one user.
