Show and Hide condition are not working properly.

The second problem has to do with Show/Hide.  When entering the form, some fields are hidden(using Form Collapse). That works nicely. But when a selction is made from the field: "How did you feel?", a previously hiddeen field should be shown. It doesn't.

I cleared the cookies, but that didn't make a difference.

Simply delete the first collapse to solve the problem:
