Why is my form not working?

    Its been about 2 years since I last created a form for one of my websites using Jot Form.

    I embedded my newly created form into the HTML source code of my website but it does not show after publishing.

    Even the link to my form (https://form.jotform.co/jsform/41639484861869 ) is not working.

     

    The message I get is FORM NOT FOUND. This form is disabled.
