Uploads in the form do not go through. Keep getting an error message upon clicking the submit button. Everything was working 2 days ago when form was tested. Kindly find attached a screenshot of the error message.

I tested your form and I was not able to replicate the problem, I was able to upload a file and submit the form. I was then redirected to the Paypal payment gateway page. Here's a screencast:
May I know if you were able to replicate the problem? Have you tried using a different browser? May we also know if you made sure that the upload process finishes before you navigated to the next page and before you submit the form?
If you have submitted the form without fully finishing the upload process, then that would cause problems and errors.
Could you please try again? if the error persists, please do provide us more information on how you have replicated the problem.

I have just tested your form just to ensure it is working fine, I can see it still does not work, I was able to replicate the problem when tried to upload more than one file, it takes some time attempting to upload then the error message is shown:
