I set up a test form on this account in June with a view to expanding it in another account after testing.
My form has completely disappeared! There is now nothing in My Forms. Am I missing something here?

Hi Andrew, there's a high possibility that you created your form last month with either a guest account or a different account.
I ran a quick check and found 2 accounts associated with your IP - andrewclark55 and cbrotary2. Both these accounts have exactly 1 form on each.

If the form you're looking for is one of the two URLs I mentioned above, then just logout of Jotform then log back in using the corresponding username.
Otherwise, if you're referring to an entirely different form, please give us the Form ID or URL so we can assist you on locating the account associated with it.
