    Why is my form disabled?

    I see that your account is auto-suspended by our anti-phishing system which is strict to scan each JotForm to calculate the probability of phishing activities.

    I have also manually checked your forms and found one of your form (formID=51413258520144) ask for the Credit card related details, it violates the terms of Jotform. So I would suggest you to remove Credit card related details from the Form, and then we will activate your account again. 

    You can create the Order or Payment form, by using the standard process of Jotform. Please check the details about Order and Payment form on the below URL:

    Your form that you are looking for is this one http://www.jotform.us/form/51413258520144.

    It seems that was the only one in violation of our terms of usage so I have removed the offending fields and reactivated your account for you.

    And please be sure in the future not to do that again or it may result in a permanent suspension.

    I recived an email with your answer. It is not my form. I think I made a mistake and I answered to a question before what was not for me.
