Hi,
I would like to know if JOT FORM CAN HELP WITH THIS:
I want customers to fill out a form, then be taken to the next page where a power of attorney form will be generated with details that they entered on the form and they can sign.
Is this possible?

You could create the form with a signature panel included:
http://widgets.jotform.com/search/signature
Then include the headers and text of your form in the PDF output:
And attach a copy of the PDF to notification and autoresponder emails.
These are our guides on setting up email notifications. The first is on setting up notifications sent to you:
http://www.jotform.com/help/25-Setting-Up-Email-Notifications
The second is how to setup notifications to your users:
http://www.jotform.com/help/26-Creating-a-Form-Autoresponder
Use the PDF attachment option to attach a PDF of your output.

They would sign directly in the form. Our signature widgets allow for collecting signatures through the form itself. They would not need to sign the PDF.
